Position: Strengthening Families Site Coordinator - Position: Part Time

TAIBU CHC, a new Community Health Centre is a population based community health centre, which is committed to providing Primary Health Care Services to the Black Community in the GTA as its priority population. TAIBU is situated in the community of Malvern and in addition to serving the diverse community of Malvern, our main focus will be on children, youth & young adults and seniors.

 

Strengthening Families Site Coordinator will be part of an inter-disciplinary team of health and social service professionals providing high quality, accessible and sustainable healthcare programs and services to the community in partnership with other community based organizations to address individual, family and community needs.

POSITION SUMMARY

 

The Strengthening Families for the Future (SFF) Program is a 14 week program for Families with Children Aged 7-11yrs lead by TAIBU Community Health Centre in collaboration with the Centre for Addiction and Mental Health. The purpose of the Strengthening Families (SF)  program is to improve family communication, build children’s strengths and life skills, increase positive and effective parenting, engage children and parents in family activities, and reduce children’s intention to use alcohol and other drugs.  

 

The SF Site Coordinator will assist with promotion of the program; provide intake services to the program and on site logistical support on the evenings the program is delivered. The time commitment of the SFF Site Coordinator will include attendance at a one day SFF training prior to the commencement of the program and during the course of the Program participation in team meetings, phone calls and email communication for the purposes of check in, planning and problem solving. 

 

RESPONSIBILITIES:

  • Promoting the Program
  • Responding to phone calls to the Intake line
  • Setting up and conducting intake interviews and ensuring Participants complete all necessary consents and documentation

  • Purchasing supplies for the Program within the budget approved by CAMH

  • Arrange for photocopying handouts for each Program session

  • Arranging for a community caterer to provide a weekly meal for the Participants within the budget approved by CAMH

  • Setting up the Program rooms and the room where the family meal will be held

  • Laying out the family meal and providing clean up following the meal each week

  • Making weekly reminder calls to the families about the upcoming program session

  • Liaising with the onsite child care service to arrange for the appropriate number of child care providers are available each week

  • Taking attendance each week of all program participants, and children in child care, and maintaining these records

  • Providing transportation support as required to SF program participants, within the budget approved by CAMH and keeping records of these disbursements

  • Liaising with facilitators and TAIBU Community Health Centre management and CAMH around issues that may arise during the course of a Program evening and participation in debriefing meetings at the end of each session

 

QUALIFICATION

 Hold of or pursuing a relevant degree from an accredited college or university (such as a post-secondary education diploma or degree in Human Services, Child and Youth Worker Diploma; Human Services Diploma; Bachelor of Social Work or Social Health Services degree) and/or an appropriate combination of education and experience.

•  Strong organizational skills

•  Excellent communication, organizational, problem solving abilities

•  Ability to work effectively in a team

•  Capable of outreaching to the African, Caribbean and South Asian communities in Malvern

•  Proven ability to build partnerships in the community

•  A member of the Malvern community a strong asset

•  Familiarity with ethno-cultural community resources in Toronto

•  Good computer skills

•  Maintains program statistics, keeps records and documentation, prepares updates for presentations

•  Available to work flexible hours including evening and weekends.

•  Proficiency in any of the  languages predominantly spoken in the community is considered an asset
